I verify that all information provided in this application forms is correct, and that as parent or legal guardian I authorize the enrollment of my child as a student at Discovery International School. I understand that failing to provide complete and accurate information may jeopardize my child's admission to or continued enrollment at Discovery International School. 私は、前述の内容に偽りがないこと、及び、前記の者がディスカバリーインターナショナルスクールの入園の申込みをしたことを証明します。前記において虚偽があった場合には、ディスカバリーインターナショナルスクールの入園取消や退園の指示に従います。私の子どもは学校の管理下にありますが、不当な目的の事故が起きた場合、ディスカバリーインターナショナルスクールも監督責任者も責任を負わないことを理解します。

If I, my spouse, or or the emergency contact persons provided in this application are unable to be reached in the event of an illness, injury, or other emergency, I authorize Discovery International School employees to determine appropriate action and/or treatment for my child. 病気、怪我、その他の緊急事態の際に、私または私が指定した緊急連絡先に連絡が取れない場合は、ディスカバリー インターナショナル スクールのスタッフが子供に対する適切な措置および/または処置を決定することを承認します。

I agree that images of my child may be used by Discovery International School for school publications and promotion (e.g., web site, print publications, advertisements).
