DIS Saturday School Application

I verify that all information provided in this application forms is correct, and that as parent or legal guardian I authorize the enrollment of my child as a student at Discovery International School. If I, my spouse, or or the emergency contact persons provided in this application are unable to be reached in the event of an illness, injury, or other emergency, I authorize Discovery International School employees to determine appropriate action and/or treatment for my child. 病気、怪我、その他の緊急事態の際に、私または私が指定した緊急連絡先に連絡が取れない場合は、ディスカバリー インターナショナル スクールのスタッフが子供に対する適切な措置および/または処置を決定することを承認します。

I agree that images of my child may be used by Discovery International School for school publications and promotion (e.g., web site, print publications, advertisements).
